Fetal and neonatal alloimmune thrombocytopenia (FNAIT) can profoundly shape pregnancy outcomes, and a case report published recently in Vox Sanguinis underscores that early testing may confirm the diagnosis but does not guarantee a favorable result.

FNAIT occurs when a mother’s immune system reacts against platelet antigens inherited from the father, most often HPAHuman platelet antigen Proteins found on the surface of platelets. Incompatibility between maternal and fetal human platelet antigens can trigger FNAIT.-1a, leading to low platelet counts in the fetus or newborn. This can result in dangerous bleeding, including intracerebral hemorrhageFetal hemorrhage Any bleeding that occurs before birth. In FNAIT, this usually results from extremely low platelet counts., which may cause death or lasting neurological injury.

“This case demonstrates the importance of HPA testing to confirm FNAIT as well as the important clinical significance of FNAIT on the outcome of pregnancy and the possibility of a poor outcome despite the implemented therapy in pregnancy,” stated the authors of this case.

Testing revealed an incompatibility in the HPA-1 system between the mother and her husband. Additional analysis identified anti-HPA-1a antibodies in the mother’s blood, confirming FNAIT. This diagnosis clarified the cause of the newborn’s complications and informed planning for a future pregnancy.
During a subsequent pregnancy, the patient received intravenous immunoglobulinIntravenous immunoglobulin A treatment given to pregnant women with a history of FNAIT or known alloantibodies. IVIG reduces the immune reaction against fetal platelets. and corticosteroids, treatments commonly used to reduce immune-mediated damage in FNAIT. Despite these interventions, the pregnancy ended at 13 weeks with intrauterine fetal death. While the exact cause was not definitively established, FNAIT was considered the most likely contributor.
For patients, this case highlights both the value and the limits of testing and treatment. Identifying FNAIT through platelet antigen testing can guide monitoring and therapy, but risks may persist. Patients with a history suggestive of FNAIT may benefit from early evaluation and specialist care, particularly in future pregnancies, to better manage this rare but serious condition.
